Understanding Malignant Obstructive Jaundice

Obstructive jaundice occurs when bile flow is blocked, commonly due to malignancy in the pancreas or bile duct. In non-operable pancreatic cancer, ERCP with metallic CBD stenting is a crucial palliative procedure that relieves jaundice, improves liver function, and enhances quality of life. Patient Presentation

A HCV-positive female patient presented to Geetanjali Multispeciality Hospital, Wai with:

  • Abdominal pain

  • Loose stools

  • Yellowish discoloration of eyes (icterus)

Clinical Findings

  • Severely deranged liver function tests

  • Markedly elevated ALP levels

  • Positive Hepatitis C Virus (HCV) status

Diagnostic Evaluation

A CT scan of the abdomen revealed:

  • Neoplastic lesion involving the head and uncinate process of pancreas

  • Metastatic deposits in liver and regional lymph nodes

  • Dilated Common Bile Duct (CBD)

  • Ascites

  • Associated acute appendicitis

Given the advanced metastatic disease, the case was deemed non-operable. The appendicitis was managed conservatively. Treatment Plan: Palliative ERCP with Metallic CBD Stenting

To relieve biliary obstruction and jaundice, the patient was planned for ERCP with Metallic CBD Stenting as a palliative intervention.
