Patient Profile


Age: 85 years

Complaint: Progressive dysphagia and inability to swallow solids

Diagnosis Tool: Upper GI Endoscopy (OGDscopy)

Findings:


  • Two grapes impacted in the lower esophagus

  • Underlying lower esophageal stricture



Procedure Performed


Endoscopic Foreign Body Removal


Tool used: Roth Net (specialized endoscopic retrieval tool)

Anesthesia: Local anesthetic spray + sedation

Outcome: Safe and complete removal of the grapes



Supportive Care


  • Ryle’s Tube insertion under direct endoscopic visualization for nutrition

  • Biopsy from the stricturous segment sent for histopathology

Through-The-Scope (TTS) Balloon Dilator (7FR)


  • Dilatation up to 12mm at 8ATM pressure

  • Immediate symptom relief and restored esophageal function



Mapping the Stricture: Image-Guided Approach


C-arm Fluoroscopy + Radiopaque Iohexol Contrast


  • Confirmed 2 cm long peptic stricture

  • Likely caused by chronic acid reflux

  • Hiatal Hernia also detected incidentally



Outcome & Follow-Up


  • Patient now tolerating oral diet

  • Repeat endoscopic dilatation scheduled in 2 weeks if required
